Instructions for New Patients

Please complete the Review of Systems form and submit or bring it with you to your initial visit

Please complete the Lifestyle Assessment and Diet diary forms and submit them 48 hours your 1 hour naturopathic visit (2nd visit)

Very often the naturopathic doctor will request a copy of your most recent blood and medical tests. If possible, please bring a copy of these to your initial or 2nd visit.
